How to become an NFTRA rider Thank you for your interest in the NFTRA riding program. To be accepted into the program, it's a requirement that you complete the Rider Contact Information Form and have your physician complete the Physician Referral Form and return both completed forms to the NFTRA at the following address: North Fraser Therapeutic Riding Association This form will then be submitted to the NFTRA medical advisory committee who will decide if the program will be suitable for you. If you're accepted into the program you will be contacted to arrange a date for a riding assessment. This assessment will provide an opportunity for you to meet the instructors and to have your needs as a rider assessed. Upon acceptance into the program you will be required to complete the following forms 1. Confidentiality and Billing and Administrative form 2. Parent/Guardian Release form 3. Emergency treatment form 4. Parent/guardian volunteer information and liability form

Inclusion in the program is dependent upon many factors, including the availability of the instructors, volunteers and horses. It's highly desirable for a parent, guardian or friend to volunteer with the rider during their lesson if at all possible.
